Catholic Charities of Buffalo announces the recent appointment of Amanda Mallwitz to a leadership role in its Care  Coordination Services.                  Mallwitz has been named  Supervisor of Project  HOPE, overseeing the staff and services  provided to keep older adults with a mental health diagnosis in their  home environment through linkage, referral, advocacy and intervention with formal and informal supports. She comes to Catholic Charities with a background in customer service, staff supervision and high quality client care.  Most recently serving as the director of community care coordination of   veterans  at Agnes Manor, she also served as a caseworker at Baker Victory Services. Mallwitz received her bachelor’s degree in psychology from Niagara University.

In addition to Project HOPE, Catholic Charities’ Care Coordination Services also provides Encompass Children’s Health Home Care Management for children birth to 21 to help meet a child’s   physical, dental or mental health needs in support of making a family’s life better, Comprehensive Care for adults age 60 and over with in-home case management and service coordination in an   effort to provide viable solutions, options and support to empower individuals to remain and thrive in their homes, and the Community Health Worker Program for individuals 18 years and older who need help navigating various healthcare systems.
