Catholic Charities of Buffalo was recently awarded $249,975 through the Community Partners of Western New York Community Based Organizations’ Innovation Fund. The purpose of the award is to create clarity in value propositions and performance capacities leading to contracts for higher performing community-based organizations with network partners and managed care organizations.

Catholic Charities will use the funding to focus on clients served by its behavioral health clinics to better understand the motivation behind frequent emergency room (ER) visits by those insured through Medicaid.

The project will help Catholic Charities determine why those clients routinely seek care through ER services rather than connecting with a primary or coordinated system of care, develop a more effective and cost-efficient service delivery model and facilitate the implementation of value based contracting with a decreased rate of ER use.

“Catholic Charities is thrilled and grateful to have been selected for this. It will support our ongoing efforts to improve service coordination and surely provide a more effective and efficient response to our clients,” said Dennis C. Walczyk, President and CEO of Catholic Charities.

“Two staff members will be added to lead the project, along with consultant support,” Walczyk said.

As the most comprehensive human service provider in Western New York, Catholic Charities makes a difference for people of every faith and ethnicity. Catholic Charities empowers children, families and seniors to achieve meaningful, health and productive lives. In all situations, Catholic Charities delivers support to meet immediate needs, and then assesses clients for other needs to ensure long-term success.
